You can save time by completing the “Pre-Registration forms” before your scheduled appointment. These forms are to be done online and does not have to be done all in one sitting (just remember your registration number and password). To access the online forms, please click the link labeled, “online patient pre-registration forms” below. Below are links to our privacy policy (HIPPA) and our financial policy. Please review all the forms below before coming in to your appointment! This will save you a lot of time on the day of your appointment! Of course, if you should have any questions, feel free to call us earlier, or our friendly staff will be able to assist you on the day of your appointment!
